Why men can no longer turn a blind eye to the early signs and symptoms of cancer





Cancer is an emerging health risk in India. As many as eight lakh new cases of cancer were diagnosed among men in this country in 2022, reveals Globocan 2022. The major cancers observed in men are lung, oral cavity, colorectum, prostate and stomach cancer. Early detection is therefore crucial to increase the chances of survival, but unfortunately most men tend to ignore the first symptoms; therefore, the diagnosis often occurs at a more advanced stage, when treatment is less effective and the prognosis is poorer. Recognizing early warning signs therefore means having an appropriate attitude towards timely medical attention.
Many of the early symptoms of the most common cancers are relatively minor or nonspecific and therefore contribute to men delaying medical care. For example, lung cancer can easily manifest with symptoms usually associated with smoking or air pollution, such as persistent coughing, shortness of breath, chest pain and unexplained weight loss. As a result, the majority of men do not visit a doctor; they treat themselves by purchasing over-the-counter medications and antibiotics and often delay their treatment. Oral cancer can cause sores, lumps, or pain in the mouth that men, especially tobacco users, may find minor annoyances. Colorectal cancer usually leads to changes in bowel habits, blood in the stool and abdominal pain; Many men view these types of symptoms as simply digestive disorders and as such are often unable or unwilling to take them seriously. In men over 50, prostate cancer manifests itself as difficulty urinating, blood in the urine or pain in the lower abdomen – all often confused with benign prostate enlargement. Stomach cancer can present as chronic nausea, vomiting, weight loss, loss of appetite and abdominal pain and is often confused with mild gastrointestinal symptoms.
One of the biggest challenges in cancer care in India is that 60-70% of cases are diagnosed at an advanced stage, when the tumor has spread to other parts of the body. For example, the stage IV diagnosis of lung cancer is not curable with currently available treatment options and survival rates are significantly reduced. Similarly, oral cancer detected at a late stage, after metastasis, may require aggressive treatment such as chemotherapy, which would then have a negative impact on quality of life. Colorectal cancers have the best prognosis if detected early, while their five-year survival decreases if these cancers have spread to other organs by the time of diagnosis. Similarly, stomach cancers diagnosed at an advanced stage also have a worse prognosis despite all the newer treatment modalities. Screening is an important part of the cancer detection process, as well as in improving outcomes. Routine screening for lung cancer is not performed in the general population, but in patients at high risk, such as with a history of smoking, previously quit within 15 years, or age ≥50 years with a 20-year history of smoking ( 1 pack/day x 20 years) – low dose CT should be offered for early detection. Oral cancer can be diagnosed during routine dental exams, especially in men who use any form of tobacco. Testing for colorectal cancer in men should begin at age 50 and earlier for men with a family history or other risk factors. This can be done by performing a colonoscopy every 10 years, along with annual stool examinations to check for blood. In most cases, prostate cancer can be detected at the age of 50 with the PSA test and DRE. Finally, men with persistent symptoms of stomach cancer, such as nausea, vomiting, or unexplained weight loss, should see a doctor. Further investigations such as endoscopy may be advised.


Early detection and timely screening, along with advanced treatments such as surgery, chemotherapy, radiation, targeted therapies and immunotherapy, can go a long way toward ensuring a cure. Since many cancers are potentially curable – if diagnosed early – or at least manageable, there would certainly be an obvious reflection in better survival rates and improved quality of life. In fact, today’s treatments – especially when combined with early diagnosis – can allow patients to live longer, healthier lives than ever thought possible.
Equally important in reducing the risk of cancer is prevention. The absence of smoking, including smokeless tobacco such as betel nut and paan, reduces the risk of both lung and oral cancer. A healthy diet and regular exercise, apart from avoiding excessive alcohol consumption, reduce the risk of colorectal cancer and stomach cancer. Regular medical checkups and routine examinations can identify potential problems well in advance before they cause cancer.
The bottom line is that the key to better cancer outcomes is found in early detection. Most malignancies that commonly affect men, including lung, oral, colorectal, prostate and stomach cancers, have symptoms that are all too easily portrayed as insignificant. Delays in diagnosis reduce treatment options and cure rates. It’s time for men to take responsibility when symptoms start to reveal themselves – whether it’s persistent coughing, unexplained weight loss or changed bowel habits – by seeing their doctor immediately. Early diagnosis and modern advances in treatment are likely to have the best chance of recovery and improvement in quality of life. Thus, regular screening accompanied by timely interventions is very important to reduce the burden of cancer and improve survival.
